Healthcare Access Maryland's (HCAM) MATCH Program works in conjunction with the Baltimore City Department of Social Services to ensure all children in foster care receive health services and ensure children's health needs are met. The Social Work Medical Case Manager provides case management for the MATCH program by collaborating with foster parents, parents, and all other members of the interdisciplinary team to coordinate holistic healthcare for children in the MATCH program.
