Sunnyside Healthcare Center is seeking a detail-oriented MDS Coordinator to lead and oversee our Minimum Data Set (MDS) process. This role is ideal for an RN or LPN with a passion for quality care, regulatory compliance, and interdisciplinary collaboration.

Key Responsibilities:

• Oversee and manage the facility's MDS process ensuring timely and accurate resident assessments
• Collaborate with interdisciplinary teams to develop individualized care plans
• Analyze resident data for quality improvement and regulatory compliance
• Maintain accurate documentation and perform audits to support MDS accuracy
• Train and support staff on MDS procedures and best practices
• Serve as a resource for regulatory updates and ensure adherence to state and federal guidelines

Qualifications:

• Valid RN or LPN license
• Previous experience in MDS coordination or long-term care settings is preferred
• Strong analytical, organizational, and communication skills
• Detail-oriented with a commitment to quality resident care
• Ability to work collaboratively with a diverse team of healthcare professionals
