The Certified Diabetes Educator provides extensive outpatient and in-patient education on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Schedule: Full-Time, Mon-Fri (Remote & In-person depending on clinical need)
Position Key Accountabilities:
1. Educate patients on blood glucose monitoring, pattern management of blood glucose, insulin administration and safety guidelines, and insulin pump and continuous glucose monitoring system training.
2. Provides assistance to the patient in obtaining diabetic supplies.
3. Refer patients to community resources.
4. Management of diabetics in the clinic, and via phone.
5. Manage the diabetic program in compliance with the American Diabetic Association guidelines.
6. Provide case management of diabetic patients under physicians' supervision.
7. Provide diabetes training and education to clinical staff.
8. Assist with research projects including data collection, patient recruitment, and other duties as needed.
9. Communicate with patients and families on a daily basis reviewing blood sugar reports.
10. Communicate with schools that have diabetic patients regarding diabetes management.
11. Refill insulin and diabetic supplies.
12. Other duties as assigned.
Certification/Skills:
Registered Nurse (RN) with current licensure or a Registered Dietitian (RD) holding active registration.
Certification as a Diabetes Educator
Minimum Education:
Bachelor's Degree in Nursing or Bachelor's Degree in Food & Nutrition Sciences or related field.
Minimum Experience:
Three years of experience. May substitute required experience with equivalent years of education beyond the minimum education requirement.
Physical Requirements:
Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
Security Sensitive:

Residency Requirement:
Employees must permanently reside and work in the State of Texas.
