Establishes new program activities and expands programs within the community in accordance with strategic and operating plans.Serve as primary program contact and respond to inquiries from the general public and members of the public health, physician and payer communities about the programs.Plan and execute Community Health wellness and educational events. Build community awareness for programs by attending health fairs, screening events, etc.Recruit, screen and register eligible participants into Community Health Programs.Enter program data into appropriate data tracking systems within appropriate time requirements.Engage marketing staff to ensure adequate publicity and marketing of programs.Support data entry, analysis, and reporting needs for community health programs and awarded grants.Effectively coordinate programming utilizing community kitchens and gardens, along with other YMCA assets, in support of overall community health strategiesMaintain relationships and communication with other Y staff at branches, facilitate regular meetings/check-ins to discuss program fidelity, challenges, and potential innovationsPerforms all other duties as assigned.
