Provide Peer Mentor services to persons assigned to caseload.
Mentoring through self-disclosure of personal recovery experience to assist with daily living skills, relationship building, identifying support networks and community resources, symptom self-management, and coping skills.
Assist in planning and implementing recovery group activities as assigned.
Work hours are flexible to meet the needs of the person served.
The majority of contacts happen out of the office and in community settings.
Contact with new referral is made within 48 hours of assignment.
ESSENTIAL JOB FUNCTIONS:
Will make all contacts as indicated in tx plan.
Weekly contact sheets turned in at the end of the workweek.
Progress notes are completed within 24 hours of service.
Will make regular contact with the treatment team.
MINIMUM QUALIFICATIONS:
Must be at least 18 years of age and have a high school diploma or equivalent.
Must identify an individual who has been in sobriety recovery of a minimum of 1 year.
Excellent interpersonal and teamwork skills
Excellent organizational and time management skills
Interest and investment in working with adults who experience Severe and Persistent Mental Illness (SPMI) and their families.
Have completed or will complete any required training within six months of employment.
Must have a current and valid Kansas driver's license.
